Welcome to St. John’s Place of Grace Church. We are honored that you choose to worship, serve, and fellowship with us. Our desire is to create a safe, welcoming environment where faith grows, community thrives, and lives are transformed.

In order to steward our ministry responsibly and protect our members, guests, volunteers, and leadership, we ask that you review and sign the following acknowledgment.

I understand that my participation in church services, fellowship meals, outdoor gatherings, ministry activities, and special events is voluntary.

These activities may include, but are not limited to:

Worship services
Fellowship breakfasts or meals
Courtyard or outdoor events (such as Easter services, baptisms, celebrations)
Ministry meetings and small groups
Volunteer service activities
Church-sponsored gatherings on or off church property

Acknowledgments

I understand that my participation (or my child’s participation) in church services, fellowship meals, outdoor gatherings, ministry activities, and special events is voluntary.

I acknowledge that participation in such activities carries inherent risks including, but not limited to, slips, falls, food-related reactions, weather exposure, or other unforeseen incidents.

I understand that St. John’s Place of Grace Church takes reasonable precautions to maintain a safe environment; however, I acknowledge that no environment can be entirely risk-free. I voluntarily assume ordinary and reasonable risks associated with participation in church activities.

If I serve in any volunteer capacity, I understand:

I am serving voluntarily.
I am not an employee.
I will not receive wages or compensation.
I am not eligible for employee benefits.

From time to time, the church may provide food and beverages prepared by volunteers or outside vendors.

I understand:

Food may contain common allergens.
I am responsible for disclosing any severe allergies for myself or my minor child.
I choose to consume food at my own discretion.

The church will make reasonable efforts toward safe handling, but cannot guarantee an allergen-free environment.

I understand that photos and videos may be taken during services and events for ministry purposes. I grant St. John’s Place of Grace Church permission to use images or video of me (or my minor child) for ministry-related purposes unless I submit a written opt-out request.

I have not been convicted of a crime that would prevent me from working with children. If asked, I am willing to submit to a background check.

To the fullest extent permitted by California law, I agree that St. John’s Place of Grace Church, its pastors, board members, volunteers, and representatives shall not be held liable for injuries or damages arising from ordinary participation in church activities, except in cases of gross negligence or willful misconduct.
